1. Requesting Your Health Care Records
- You have the right to access and obtain copies of your records maintained by this practice.
- Submit a written request including:
- Full name
- Date of birth
- Contact information
- Records requested
- Requests may be submitted via mail, email, or client portal.
- We process requests in accordance with Texas and federal law.
